Exactly like AddictPunk said. I prefer to automate the main volume on the stereo output device, because then I'm sure to catch all the audio and also have an exclusive automation track only for that purpose. You can also experiment with the shape of the automation curve to get different fades. I like to start the fade out fairly quickly and finish it slower.
